100 Point Identification Check

The 100 Point indentification check must be completed upon registration with E4 Recruitment.

Please see below the point scoring system and what is acceptable to submit. The point score of documents produced must total to at least 100 points and must include at least one form of photo ID.

The following documents must have a PHOTOGRAPH and NAME:


  • Driver License issued by an Australian State or Territory

  • License or permit issued under a law of the Commonwealth, a State or Territory Government (e.g a boat license)

  • Identification card issued to a public employee

  • An identification card issued by the Commonwealth, a State or Territory as evidence of the applicant's entitlement to a financial benefit

  • An identification card issued to a student at a tertiary education institution

  • A proof of Age Card or NSW Photo Card issued by the NSW Roads and Traffic Authority

The following documents must show NAME and ADDRESS:

  • A document held by a cash dealer giving security over the applicant's property

  • A mortgage or other instrument of security held by a financial body

  • Council rates notice

  • A document from the applicant's current or former employer within the past 2 years

  • A document from the Credit Reference Association of Australia

  • Land Titles Office record

  • Marraige Certificate (for maiden name only)

  • Credit Card (only sighted and photocopied, card number should be blacked out on copy before filing)

  • Membership card for a registered club

The following documents must show NAME and ADDRESS:


  • The electoral roll compiled by the Australian Electoral Commission and available for public scrutiny

  • A recent signed reference of recommendation for an acceptable referee (e.g. doctor, teacher, clergy, banker, police, jusice of the peace etc.)

  • Lease/Rental Agreement

  • Rent receipt from a licensed real estate agent

  • Record of a public utility (e.g. telephone, water, gas or electricty bill)

  • Records of a financial institution

  • A record held under law other than a law relating to land titles

  • Foreign Driver's License

The following documents must show NAME and DATE OF BIRTH:


  • The records of a primary, secondary or tertiary institution attended by the applicant within the past 10 years

  • The records of a professional or trade association of which the applicant is a member

Working with Children Checks

A Working with Children Check is a review of findings of misconduct involving children and the result is either a clearance to work with children for five years, or a bar against working with children. If the outcome is a clearance, the Check can be used for any child-related work, or work in a medical field, for a period of five years. This Check is mandatory.

Who needs a Working with Children Check?

Any individual that works in a child-related or medical work must apply for a Working with Children Check. Child-related work means either direct or indirect contact with children and/or their records in a child-related sector, or work in a stipulated, child-related role.

Current Working with Children Checks

If you hold a current Working with Children Check, please have this certifed by a Justice of the Peace and post directly to your E4 Recruitment Recruitment Consultant.

Please note each state and territory have their own procedures and it is necessary to fulfill the requirements of the jurisdiction(s) in which you are working. Some Hospitals may ask you to complete a state specific Working with Children Check. This instruction will be lead by your Recruitment Consultant/administration at the time of booking.

Please note: this is your individual check at your own expense and will be your responsibility to maintain and manage.

National Criminal Record Checks

It is the responsibility of the individual to withhold a National Criminal Record Check. This is a mandatory requirement to assess the suitability of an individual for employment.

Once the NCRC has received clearance, you will be required to submit an original certified copy of the certificate to E4 Recruitment. Or if your NCRC can be validated online, you do not need to have this certified.

For individuals registering with E4 Recruitment who already hold a NCRC from within the past 3 years, you will be required to be present a Statuatory Declaration regarding any further matters since the date of the NCRC.

Vaccination Declaration

E4 Recruitment is dedicated to ensuring the health and safety of all our candidates in health care settings and in providing a safe and healthy working environment for all staff and patients. 

We therefore have an assessment, screening and vaccination policy that minimises the risk of acquiring infectious diseases. Please ensure you send us your evidence along with this signed declaration.

Evidence Required to demonstrate protection against the specified infectious diseases:

1. Acceptable evidence of protection against specified infectious diseases includes:

  • A written record of vaccination signed by the medical practitioner, and/or

  • Serological confirmation of protection, and/or

  • Other evidence as specified in the table below

  • NB: the health facility may require further evidence of protection, e.g. serology, if the vaccination record does not contain vaccine brand and batch or official certification from vaccination provider (e.g. clinic/practice stamp.

2. TST screening is required if the person was born in a country with a high incidence of TB or has resided for a cumulative time of 3 months or longer in a country with a high incidence of TB.

3. In certain clinical settings, for example in an oncology or neonatal ward, the health facility may require serological evidence of protection in addition to evidence of vaccination or other evidence to ensure that the risk to vulnerable patients is minimised.

Influenza - annual influenza vaccination is not a requirement but is strongly recommended.

Note: interferon-gamma release immunocassay (IGRA) is not general accepted. In the event that an IGRA has been performed, screening by TST will be required if the IGRA result is negative or equivocal.

Persons with positive TST/IGRA must be fully assessed by a TB service within 3 months of commencement of clinical duties or clinical placement and must be asymptomatic when commencing clinical duties or clinical placement.
